  • WHEN COMPLETING THIS FORM:
    1. Before you start, please be ready to provide the date and place of your child's baptism.
    2. The Sponsor/Godparent's name, address, phone, email & current parish.

    Students are required to complete the following activities for Confirmation preparation and the sacrament:

    • Enrollment Mass: November 5, 2025 @ 7:00 PM in the church. (Please sign-in at 6:00 PM to avoid long line-ups.)
    • Online Sessions: Attend six sessions via The Mark (link will be emailed).
      • Session 1 Release Date: December 13, 2025
      • Session 2 Release Date: December 27, 2025
      • Session 3 Release Date: January 11, 2026
      • Session 4 Release Date: January 25, 2026
      • Session 5 Release Date: February 08, 2026
      • Session 6 Release Date: February 22, 2026
    • Retreat: Choose one date (limit of 60 students/day):
      a. Date: Saturday March 07, 2026 from 8:30am-3:30PM
      b. Date: Saturday March 08, 2026 from 8:30am-3:30PM
    • Sacrament of Confirmation (by school schedule):
      a. St. Brendan & St. Jean de Brebeuf on Sunday May 03, 2026 @ 1PM
      b. Cardinal Leger, St. Dominic Savio & Sunday School on Sunday April 26, 2026 @ 1PM

    IMPORTANT: Candidates who fail to participate in any of these events without a valid reason may be required to withdraw from the program and delay Confirmation till another time. If you decide that this is not the time for you to celebrate the Sacrament of Confirmation, you are asked to contact the parish office at 416.282-0370. We will respect your decision, and encourage you to consider being confirmed when the time is right for you.

  • Parents' Information

  • For our Confirmation Preparation programme, we will be enlisting the company Hopestone to provide a virtual confirmation preparation programme for your children.

    The online programme is called "The Mark" and can be previewed using this link: https://www.hopestone.ca/confirmation

    This 6-session online course will be released weekly, one session at a time. Each 20–30 minute session includes a prayer, a talk on confirmation, and a personal testimony.

  • Confirmation Sponsor Form

  • Please make sure that the sponsor is eligible according to the following:

    Canon 892: Insofar as possible, there is to be a Godparent for the person to be confirmed; the Godparent is to take care that the confirmed person behaves as a true witness of Christ and faithfully fulfills the obligations inherent in this sacrament.

    Canon 893: To perform the function of Godparent/Sponsor, a person must fulfill the condition mentioned in Canon 874 (see below).

    Canon 874: The following are the requirements in order for a Catholic to be a Godparent/Sponsor:

    • At least 16 years of age.
    • She/He has been fully initiated in the Catholic Church (received Baptism, Holy Communion, and Confirmation).
    • In good standing with the Catholic Church: live a life of faith, which benefits the role to be undertaken; not under canonical penalty.
    • Not the Father, Mother, Step-parent or Guardian of the one to be Confirmed.

    It is recommended that the sponsor be one fo the Candidate's Baptismal Godparents.

I further declare that the information provided on this page to be true and accurate to the best knowledge and that I am eligible to be a Sponsor based on the requirements of Canon 874 as stated above.   
    (On Confirmation Enrollment Night, you will receive a form that your sponsor needs to sign. Please note that the sponsor does not need to be present that evening.)

  • Contribution to Cost of Materials

    The Sacrament of Confirmation is a free gifts of Christ through His Bride, the Church. However, we are asking for a contribution of $30 to help us recover the cost of programme and certificates. If the amount presents an extreme financial burden on you, any amount you can afford will be appreciated. On the other hand, if you are financially able to donate for another family, we welcome your donation, and may God bless you a thousand-fold for your generosity.
