I understand that by enrollment in Empire Youth Athletics programs, permission is granted to use any photograph in connection with Empire Youth Athletics for publicity and/or media purposes. I understand that participation in the Activity involves inherent risks, including risk of physical injury and I assume all related risks and voluntarily participate in the Activity. I understand Empire Youth Athletics is not liable for any contraction of Covid-19. This includes any workouts at 423 Park Ave, Huntington, United States. The property is not responsible for any related risks. I attest that: * I/my child am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell. * I/my child have not traveled internationally within the last 14 days. * I/my child have not traveled to a highly impacted area within the United States of America in the last 14 days. * I/my child do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. * I/my child have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities. * I/my child am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
