          • Parent/Guardian Liability Waiver & Code of Conduct Agreement

This must be signed before a student may participate in any Cultivating Kids (“CK”) programs.

            1. Acknowledgment of Risk
I, the undersigned parent/guardian, acknowledge that participation in photography, storytelling, media, and related CK activities may involve certain risks, including but not limited to: use of equipment, movement between activity spaces, and participation in group discussions. I understand that reasonable measures are taken to ensure safety but recognize that accidents or injuries may still occur.

            2. Release of Liability
I release and hold harmless Cultivating Kids, its directors, instructors, volunteers, and partner schools from any claims, damages, or liabilities for personal injury, property damage, or other loss, except in cases of gross negligence or willful misconduct.
I assume full responsibility for any medical expenses that may result from participation.

            3. Medical Authorization
In case of emergency, I authorize CK staff to obtain medical care for my child if I cannot be reached. I release CK from liability for medical treatment decisions made in good faith.

            4. Student Work & Use Rights
All creative work produced by students during Cultivating Kids programs (including but not limited to photographs, videos, writings, and artwork) remains the intellectual property of the student. By participating, students and parents grant Cultivating Kids a perpetual, royalty-free license to use, reproduce, display, publish, and distribute such work for educational, promotional, fundraising, and marketing purposes. This license does not restrict the student’s own use of their work, and students are encouraged to share, publish, and build upon their creations outside the program.

            5. Code of Conduct
Cultivating Kids is committed to forming not only skills but also character. All participants are expected to uphold standards consistent with classical school values, including:

Respect for instructors, peers, and property.
Conduct marked by truth, beauty, goodness, and excellence.
Participation in thoughtful discourse and willingness to learn from feedback.
Prohibition of disrespectful language, disruptive behavior, or misuse of equipment.
CK reserves the right to dismiss a student from the program if conduct consistently violates these values. No refunds will be issued in such cases.

            6. Acknowledgment & Agreement
By signing below and submitting this registration, I acknowledge and agree to these terms.
