By signing this agreement, I acknowledge and agree to the following terms and conditions:
Non-Refundable Deposit: I understand that the deposit I am paying is non-refundable under any circumstances except where the tattoo artist or studio is unable to fulfill the appointment due to unforeseen events. If I cancel or fail to show up for my appointment, the deposit will be forfeited.
Rescheduling Policy: I understand that if I need to reschedule my tattoo appointment, I must provide a minimum of 48 hours’ notice. If I fail to reschedule within this time frame, the deposit will be forfeited. In the case of a reschedule, the deposit will be applied to the new appointment date, subject to availability.
Health and Safety: I confirm that I am in good health, free from any conditions that could affect the tattoo process or healing (such as allergies, skin infections, or blood disorders). I have provided accurate information regarding any medical conditions or medications that may affect my tattoo. I agree to notify the tattoo artist immediately if any changes to my health arise before or during the appointment.
Age Requirement: I certify that I am at least 18 years of age and legally capable of receiving a tattoo. If I am under the legal age in my jurisdiction, I will provide written consent from a legal guardian in accordance with state laws.
Design and Modification: I understand that the tattoo design may be subject to changes based on the artist’s discretion, and the artist reserves the right to refuse any design or modification that they believe is inappropriate, unsafe, or unworkable. I acknowledge that final design approval must be provided at least 48 hours before the scheduled appointment.
Tattoo Studio's Right to Refuse Service: I acknowledge that the tattoo artist and/or studio reserves the right to refuse service at any time for reasons including but not limited to inappropriate behavior, intoxication, or failure to comply with studio policies.
Video Surveillance Notice: Our premises are monitored by continuous video recording systems operating 24 hours a day, 7 days a week, for security, safety, loss prevention, and operational purposes. By entering our property or using our facilities, you acknowledge that you have been notified of and consent to video recording in accordance with applicable law.
Healing and Aftercare: I understand that aftercare is critical to the proper healing of my tattoo. I agree to follow the tattoo artist’s instructions carefully. I acknowledge that the studio is not responsible for any complications arising from improper aftercare or failure to follow the artist's instructions.
Privacy and Media Rights: I give the tattoo artist and the studio the right to photograph and/or record the tattoo process for promotional and marketing purposes. I understand that my likeness, tattoo, and video/photos may be used on the studio’s website, social media, or other platforms unless otherwise stated.
Liability Waiver: I, the client, understand that receiving a tattoo involves potential risks, including infection, allergic reaction, or scarring. I accept full responsibility for these risks and release the tattoo artist and studio from any claims, damages, or liabilities arising from the tattoo process, healing, or any complications related to the tattoo.
Agreement Binding: I acknowledge that this agreement constitutes the full and complete understanding between the parties. Any modifications to this agreement must be made in writing and signed by both the client and the tattoo studio.
