Hoopers Academy Basketball for Girls

I, the parent/guardian of the participant, hereby give my permission for my child to participate in the basketball skills program offered by Hoopers Academy at Tuxedo Park School. To the best of my knowledge, my child is physically fit and able to participate in these activities. I understand that there are inherent risks involved in any physical activity, and I agree to assume all responsibility for any injuries or damages that may occur to my daughter or myself as a result of her participation. I also agree to waive, release, and hold harmless Hoopers Academy, its staff, volunteers, and affiliates from any claim arising out of any injury to my child(ren), whether the result of negligence or for any other cause, except to the extent and in the amount covered by accident or liability insurance. I understand and assume all risks and hazards associated with participation in these activities, including the transportation to and from the activities, and do hereby waive, release, absolve, indemnify, and agree to hold harmless to the extent permitted by law, the Tuxedo Park School, The Tuxedo Club, Village of Tuxedo Park, organizers, sponsors, participants, and all persons and companies providing such activities and transporting my child(ren) to or from activities, from any claim arising out of any injury to my child(ren), whether the result of negligence or for any other cause, except to the extent and in the amount covered by accident or liability insurance. In the case of a medical emergency, I authorize the Hoopers Academy representatives to seek medical treatment for my child at the nearest medical facility and they may call paramedics and discharge my child to an ambulance if I am not able to authorize it at the time of the medical emergency.
