Middle School Division: Starts Tuesday, January 27th • 5 weeks (Jan 27 – Feb 24) • Sand Pine Park

6th-8th Grade Tuesday 6:30–8:30 PM

Format: 4 games per division per night (20-minute games). Each team plays at least twice weekly; rotating double-headers.
Price: $50 per player (max 5 players/team, 5 teams/division).

• Lightning/Thunder: pause immediately; if not clear in 30 min, reschedule.
• Heavy rain: postponed for safety.
• Make-ups: end of season or available weekday.
• Updates: text/email 1 hour before game time.

SK Academy 3v3 Basketball League – Waiver & Release

By completing this registration, I acknowledge and agree to the following:
 1. Assumption of Risk
I understand that participation in basketball involves inherent risks, including but not limited to falls, contact with other players, and weather conditions. I voluntarily assume full responsibility for any risks of injury or loss.
 2. Medical Release
In the event of injury or medical emergency, I authorize SK Academy staff to administer first aid and/or arrange for medical treatment for my child as deemed necessary. I accept full responsibility for any resulting medical expenses.
 3. Waiver of Liability
I hereby release and hold harmless SK Academy, its coaches, staff, volunteers, and facility partners from any and all liability, claims, or demands arising out of my child’s participation in the 3v3 Basketball League.
 4. Code of Conduct
I agree that my child will follow all league rules, respect officials, coaches, and other players, and demonstrate good sportsmanship at all times. I understand that unsportsmanlike behavior may result in suspension or removal from the league without refund.
 5. Photo/Media Release
I grant SK Academy permission to use photographs, video, or likeness of my child for promotional and educational purposes, without compensation.
