Luna y Cielo Day Off Camp 2025-26 Enrollment Form

Our Day Off Camps will be fully Spanish immersion.  Our goal is to expand the language for students who are dominant in Spanish, currently enrolled in a dual language program or are enrolled in Luna y Cielo's Spanish programs. We will be speaking Spanish during our activities and promoting Spanish during independant play. 

8:30 AM – 9 AM: Kids Arrive for Morning Care / Independent Play $5 extra
9:00 AM- Regular camp begins

 
10:30 AM: Bathroom & Snack/Time Clean-Up Time (Kid Participation)

 
10:40 AM – 12:00 PM: STEAM projects

 
12:00 PM – 12:45 PM: Lunchtime

 
12:45 PM – 1:00 PM: Clean-Up Time (Kid Participation)

 
1:00 PM – 2:30 PM: Independent Play

 
2:30 PM – 2:50 PM: Clean Up & Bathroom Break

 
3:00 PM-Regular Camp ends Aftercare $15/day

3:15 PM – 4:30 PM: Cooking Class / Snack Making

4:30 PM – 6:00 PM: Activity & Clean up

• I, the parent/guardian of this camper, gives authority to the staff of this camp to apply judgment in regards to medical assistance in the event of an accident, injury, or illness if the emergency contact person cannot be reached. I authorize first aid, medical/surgical diagnosis, and treatment which may deem necessary.
• I released the organizers, staff or Luna y Cielo LLC for any responsibility in case of accident, illness, death or injury, during my child's enrollment.
• I confirm that all information given in this form is true, complete, and accurate.
• I acknowledge that my child is fully potty trained and will not expect the Luna y Cielo staff to change my child if they have an accident. If it becomes apparent that my child is not potty trained, I acknowledge that my child  will not be permitted to return and no refund or credit will be issued. 
• I acknowledge that Luna y Cielo cannot accommodate naps and will not expect a refund if Luna y Cielo is unable to accommodate my child(ren)'s nap. 

In Consideration of being allowed to enter the play area and/or participate in any program, activity, and/or trip to the park at or with Luna y Cielo Play Cafe, the undersigned, on the behalf of the participant(s) identified below. acknowledges, appreciates and agrees to the following conditions:

I represent that I am the parent or legal guardian of the participant(s) named above, or have obtained permission from the parent/legal guardian of the participant(s) named above to execute this agreement on their behalf. I agree that the participant(s) named above and I shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for participation in any program, activity, and/or trip at or with Luna y Cielo Play Cafe. 

I am aware that there are inherent risks associated with my child's participation at Luna y Cielo Play Cafe.  On behalf of myself and the participant(s) named on the above knowingly and freely assume all such risk, both known and unknown. including those that may arise out of the negligence of other participants.

I for myself and the participant(s) named above. and our respective heirs, assigns. administrators. personal representatives, and next of kin, hereby release and hold harmless, Luna y Cielo Play Cafe, and Luna y Cielo LLC's management, their affiliates, officers, members, agents. employees, other participants, and sponsoring agencies from and against any and all claims, injuries. liabilities or damages arising out of or related to our participation in any and all Luna y Cielo Play Cafe program, activity, and/or trip and the use of the play area.

Luna y Cielo Play Cafe has my permission to use my or my child's photograph publicly to promote Luna y Cielo. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

Our primary goal at Luna y Cielo Play Cafe is to provide a safe, positive, and enjoyable environment for all participants. By signing below you understand that Luna y Cielo staff reserves the right to dismiss your child from camp if your child's behavior poses a threat to themselves or others at any point during our weekly camps. No refunds will be provided in such cases.

If you withdraw from a camp five calendar days or more from the camp start date you will have the following options:

Option 1: Request a Program Credit. Credit the customer’s account with the balance for a future class, camp, special event, or program. Credits must be used within 6 months (updated 8/22/25).

Option 2: Transfer camp dates to another available date. We do not give refunds for illness or absences after the five day period.
