LIABILITY WAIVER
In consideration of being permitted to use the facilities, equipment, and premises at Triumph Barbell, I, the undersigned, on behalf of myself, my heirs, executors, administrators, successors, and assigns, hereby agree as follows:

1. ASSUMPTION OF RISK AND RELEASE OF LIABILITY
I understand that the use of fitness equipment, weightlifting, and participation in gym-related activities involves inherent risks of physical injury, including but not limited to muscle strains, sprains, broken bones, permanent disability, and death. These risks may arise from my own actions or inactions, the actions or inactions of others, or the condition of the facilities or equipment.

I voluntarily assume all such risks and agree that Triumph Barbell, its owners, employees, agents, affiliates, and insurers shall not be liable for any injuries (including death), damages, or losses sustained by me or my guest(s), including any minor(s) under my supervision, whether caused by negligence or otherwise.

2. RELEASE AND WAIVER
I hereby fully and forever release, discharge, and hold harmless Triumph Barbell, its owners, employees, agents, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to my (or my minor child’s) participation in gym activities or use of equipment, whether occurring on the premises or as a result of travel to and from the facility.

3. INDEMNIFICATION
I agree to indemnify and hold harmless Triumph Barbell from any and all claims brought by or on behalf of any person (including minors) arising from injuries or damages sustained by me or my child in connection with our use of the facility.

4. USE OF FACILITIES AND RULES
I understand that Triumph Barbell may not provide supervision, instruction, or assistance during use of its equipment or facilities. I agree to follow all rules and regulations set forth by Triumph Barbell and to use the facilities and equipment only in a manner consistent with their intended purpose. I accept full responsibility for my conduct and that of any minor(s) accompanying me.

5. PERSONAL PROPERTY
I understand and agree that Triumph Barbell is not responsible for any lost, stolen, or damaged personal items.
By signing below, the Member affirms they have read, understood, and agree to be bound by the terms of the Triumph Barbell Liability Agreement.*
