HOLD HARMLESS, RELEASE OF LIABILITY & ASSUMPTION OF RISK AGREEMENT
I, the undersigned parent or legal guardian, hereby give permission for my child to participate in the Youth Musical Arts Academy of North Port (YMAANP) Summer Band Camp and any related activities, rehearsals, performances, and events (collectively referred to as “the Program”).

1. Assumption of Risk
I understand that participation in music education activities, including but not limited to rehearsals, performances, movement, instrument handling, and transportation within camp facilities, involves inherent risks of injury or illness. I voluntarily assume all such risks on behalf of my child.

2. Release and Hold Harmless
In consideration of my child’s participation in the Program, I hereby release, waive, discharge, and hold harmless the Youth Musical Arts Academy of North Port, its officers, directors, employees, volunteers, instructors, contractors, sponsors, partner organizations, and facilities from any and all claims, demands, actions, or causes of action arising out of or related to my child’s participation in the Program, including but not limited to claims for personal injury, illness, property damage, or loss, whether caused by negligence or otherwise, to the fullest extent permitted by Florida law.

3. Medical Authorization
In the event of illness or injury, I authorize YMAANP staff or representatives to obtain emergency medical treatment for my child if I cannot be reached. I understand that I am financially responsible for any medical care provided.

4. Health and Participation
I certify that my child is physically and emotionally able to participate in the Program and has no medical conditions that would prevent safe participation, except as disclosed in writing to YMAANP prior to the start of camp.

5. Code of Conduct
I understand that my child is expected to follow all Program rules, safety guidelines, and staff instructions. Failure to do so may result in dismissal from the Program without refund.

6. Governing Law
This agreement shall be governed by and construed in accordance with the laws of the State of Florida.
7. Photo and Media Release I grant permission for YMAANP to photograph or record my child during Program activities and to use such media for educational, promotional, or marketing purposes without compensation.*

I grant permission for my child to be photographed or recorded.I do not grant permission.
I have read this agreement carefully, fully understand its contents, and voluntarily agree to its terms on behalf of myself and my child.*
