Waiver, Release, and Consent Agreement
I, the parent/legal guardian of the registered athlete, hereby acknowledge and agree to the following terms:
Assumption of Risk
I understand that participation in the Vision Elite Tryout involves physical activity and carries a risk of injury. I assume full responsibility for any injuries or damages that may occur during participation.
Medical Consent
I authorize the event staff, coaches, or medical professionals to provide emergency medical treatment to my child if I cannot be reached. I agree to bear any costs associated with such treatment.
Waiver and Release of Liability
I release and hold harmless Vision Elite Athletics LLC, Hoop Connect, event staff, directors, volunteers, officials, and facility partners from any liability, claims, or damages arising from participation in this event.
Photography and Videography Release
I grant permission for photographs and video footage of my child to be taken during the event. I understand that these may be used for marketing, promotional, and educational purposes on websites, social media, flyers, or other publications related to Connect Basketball League or its affiliates. I waive any rights to compensation or ownership.
Accuracy of Information
I certify that the information provided in this registration form is accurate and truthful. I understand that any false information may result in disqualification from the event.
