Safety Waiver to Participate

IMPORTANT NOTE: You are completing this waiver for yourself and all additional people you registered in this form.

WILLFUL PARTICIPATION AND RELEASE FROM LIABILITY AND INDEMNIFICATION

I, {name}, am a willing participant in the UNW Purple Donut Run at University of Northwestern-St. Paul on October 11, 2025.

I acknowledge that the varied terrains of the UNW Purple Donut Run event may include, but are not necessarily limited to: hills, rough terrain, public streeets, private asphalt driveways, asphalt streets, asphalt and concrete parking lots, grass, sand, and dirt.

By signing this document, I acknowledge that there are hazards involved in participating in the Event(s), which may include but are not limited to, travel to and from the Event(s), travel in vehicles, physical exertion, and exposure to disease, sickness, and venomous animals. In addition, since some activities may be conducted in remote areas, medical care and access to medical facilities may be restricted. By signing this document, I assume the risks of these and other hazards and agree I will not hold University of Northwestern – St. Paul | Northwestern Media (UNW), its directors, trustees, offices, employees or agents responsible in the event any one or more such hazard occurs.

I am a willing participant and acknowledge and agree that in consideration of being allowed to participate in this/these Event(s), I am assuming the risks involved, and I, on behalf of myself, my heirs and assigns, hereby release UNW, its directors, trustees, officers, employees or agents from any and all liability, loss, cost or damages (collectively "Liabilities"), arising from or related to my participation in the Event(s), including any such liabilities resulting from any negligence or fault of the Participants, active or passive, except that this release will not apply to intentional, willful, or wanton acts of UNW, its directors, trustees, officers, employees or agents.

I further agree to indemnify and hold harmless UNW, its directors, trustees, officers, employees or agents for all costs, damages, and/or awards, including attorneys fees, arising out of any demands, causes of action, claims, counterclaims, or cross claims brought by any other person or entity against UNW, its directors, trustees, officers, employees or agents, pertaining to, arising out of, or as a result of my acts or omissions, whether intentional or negligent, in connection with the Event(s).

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY, INDEMNIFICATION AND A CONTRACT BETWEEN UNW AND MYSELF. I HAVE SIGNED OF MY OWN FREE WILL.
