I acknowledge that I will read and share with my student participants/parents the NC FPS & FPSPI Rules and Regulations as well as the NC FPS Code of Conduct after receiving my GIPS Coach Materials.I will ensure that my student participants and their parents will read and electronically sign the NC FPS Code of Conduct as soon as reasonably possible, and I will keep that signed form on file in the event that the NC FPS Board of Directors or NC FPS Appeal and Review Committee requests a copy of that signed document.
