I, the parent/guardian of the registered participant, understand that participation in the Carolina Smoke Basketball Club Summer Camp involves physical activity and the potential risk of injury. I hereby release and hold harmless Carolina Smoke Basketball Club, its coaches, staff, volunteers, and facility partners from any and all liability, claims, or damages arising from participation in camp activities. I certify that my child is physically able to participate and authorize camp staff to seek emergency medical treatment if necessary.
Media Release Authorization

I, the undersigned parent or legal guardian of the above-named minor athlete, hereby grant permission to Carolina Smoke Basketball Club (CSBC), its representatives, employees, volunteers, sponsors, partners, and authorized media personnel to photograph, videotape, livestream, record, and otherwise capture the likeness, image, voice, name, and participation of my child during practices, games, tournaments, camps, clinics, team events, fundraising activities, and other club-related functions.

I understand and agree that these photographs, videos, recordings, and other media may be used by Carolina Smoke Basketball Club for lawful promotional, educational, recruiting, fundraising, and marketing purposes, including but not limited to:
