Please will out this form completely to register your dancer for SDDT's Nutcracker Auditions. Please submit a seperate form for each child. Thank you!
SDDT Nutcracker Auditions: Saturday 9/6/25

Ballet 1 and Non-Ballet Students - 1:30pm-2:00pm
Ballet 2 & 3 - 2:00pm-2:30pm
Ballet 4+ - 2:30pm-3:00pm

All who audition will be given a role!

South Dayton Dance Theatre (a not for profit organization) is pleased to offer a professional performing opportunity, in their production of The Nutcracker Ballet, to all South Dayton School of Dance students placed in Ballet 1+ as of 9/6/25, and free to commit to the entire mandatory rehearsal schedule.

During your child's Nutcracker Audition time slot there will be a Mandatory Parent Meeting. During this meeting, we will discuss the requirements and commitment required to participate, and also answer any questions you may have.

Please be informed that ALL Nutcracker Rehearsals are Mandatory! If your child is currently involved in other activities,we ask that you consult your schedules (prior to audition day) to be certain that you will be able to fulfill an additional commitment. Dancers are permitted 1 excused absence from a scheduled nutcracker rehearsal given that the Aritstic Staff are notified on this form PRIOR to your dancer auditioning (may not be used for an in-studio dress rehearsal, dress rehersal at the theater, or a performance). Any other absence from a scheduled Nutcracker rehearsal will result in your child's role being immediately & permanently reassigned to another student.

A $100.00, per student, audition fee is to be paid at time of audition. This fee now includes 1 Nutcracker T-shirt per dancer. Audition Form & $100 Fee must be completed and submitted prior to the time of audition to be considered for a role in the 2025 Nutcracker Ballet (online form to be submitted no later than Friday 9/5 at 11:59pm).

Payment must be made online via PayPal using this form. We are not accepting checks or cash for this audition. Thank you for your understanding.

Waivers

Waivers

The following waivers must be signed for participation
I, the undersigned, give my permission for the above mentioned dancer to participate in SDDT's Nutcracker audition process, rehearsals, and performances. I recognize the possibility of physical injury and risk of illness to the student associated with taking part in this activity. Upon signing this waiver, release and forever discharge any and all rights and claims for damages, which may arise now or in the future against SDDT and its assigned representatives or volunteers from any and all liability and for any and all damages, illness and/or injuries which may be sustained or suffered by the student while participating in the SDDT Nutcracker Audition, rehearsals or performances. Furthermore, in the event SDDT is unable to contact the authorized person(s) on this form, I the parent or legal guardian, hereby give my consent for emergency medical care. Transportation to the hospital will be at the discretion of the Emergency Technicians on site. *

I, the undersinged, hereby grant to the South Dayton Dance Theatre all rights of every kind to use my child's photographs or films. I agree that SDSD and SDDT has the right to record the material for use on all formats now known and yet to be discovered. I agree to indemnify and hold SDSD and SDDT, its agents and officers harmless against liability associated with the distribution and/or transmission of any recorded media. I fully understand that all above recorded materials may be used now and in the future by the South Dayton School of Dance or South Dayton Dance Theatre.*
