Required for Private Lesson Participation
By submitting this form, I agree to the following terms regarding private cheer lessons provided by Five Star Athletics:
Lessons are 30 minutes in duration and must be scheduled in advance.
Full payment is required at the time of booking.
Late arrivals may result in shortened lesson time. Lessons will end at the originally scheduled time.
There are no refunds for any reason, including missed or canceled lessons. Rescheduling requires at least 24-hour notice and is subject to availability.
Five Star Athletics reserves the right to substitute coaches when necessary.
Assumption of Risk:
I understand that cheerleading, tumbling, and stunting involve physical activity and carry inherent risks of injury.
I voluntarily assume all risks associated with participation and agree not to hold the coach, Five Star Athletics, or any of its volunteers, staff or affiliates liable for any injury or accident that may occur during lessons.
Participant Health:
I confirm that the participant is physically able to participate in cheer-related activities and does not have any medical condition that would prevent safe involvement.
Media Release:
I authorize the use of photographs or videos taken during lessons for Educational and promotional purposes.
Parental Consent (if under 18)
As the parent/guardian, I give consent for my child to participate in private cheer lessons and agree to all terms stated above.
