    These forms are required for your children to attend camp! New for this year, camp will be held July 13 - 17, 2026 at Crestline Elementary School located at W Jackson Blvd, Mountain Brook, Alabama. If you are registering more than one child, you will need to submit a separate registration form for each child.

  •  Parent/guardian cannot be listed as emergency contacts. List the name of at least one person who can be contacted in the event of an emergency or illness if you cannot be reached. Any person listed should be able to assist in contacting you. At least one person listed must be able to take responsibility for the child in case the parent/guardian cannot be reached. 

  • Any Additional Approved Pickup Names

  • Only the parent/guardian, emergency contact(s), or the names on the approved pickup list will be able to pick up the camper. You will be required to show valid photo identification in order to pick up the camper.
