Along with this form you can also download the required documents (or email them to the Faith Formation Office), make your electronic payment and schedule your One-on-One meeting with our coordinator. If your child is unbaptized, our office requires a copy of their state birth certificate. Please take a few moments to fill out this registration form. Thank you!
