Terms & Conditions + Liability Waiver* This field is required.It is understood that I (signature below) am agreeing to pay ‘Coach Greyson Inc.’ the total contractual amount of the introduction session. Payment must be submitted by or before the introduction session. After confirming the introduction session and payment is arranged, there will be no refunds. I (signature below) am granting full permission for my child or myself to participate in the introduction session. I fully understand that should any major medical issue or accident occur during training or workouts that the strength & conditioning staff will notify me. In addition to this liability, I am giving the strength & conditioning staff release of all claims on account of any injuries which may accidentally occur during the introduction session. I am fully aware of the risks involved with resistance training & conditioning, plus the use of the gym and/or their equipment. I am aware and acknowledge such risk on behalf of me or my child while undergoing the introduction session. I also agree to indemnify ‘Coach Greyson Inc.’ and its staff of any claims, from here on may be presented to me or my child as a result of any injury. I also am granting the strength & conditioning staff the use of photography and videography of me or my child for marketing/advertising/other commercial uses, while also utilizing it as a valuable coaching tool. This in entirety admits all qualified applicable participants without regard to sexual orientation, race, disability, or religion. I am fully aware of all that is stated above and will certify my understanding by authorizing with my signature below.
