  • Section 1 — Student Information

Note: Grade refers to the grade your child will be entering August 2026-2027.

Your child's name, grade, and age are required information.  Information provided on this registration form must match information provided during our Information sessions and Open Houses at check-in.

    Special Learning Needs

If your child has special learning needs, please stop here and schedule a consultation before completing this form.

Our goal is to ensure that every student we enroll can thrive in the Aspire learning environment. A consultation allows us to better understand your child’s needs and determine whether our program is the best fit and whether we can provide the appropriate support.

If a consultation is needed, please do not proceed to the payment section until we have spoken with you.

  • SECTION 4 — Medical Information

Does your child have any medical conditions we should be aware of?

  • Section 5 — Allergies & Health Disclosure
Please list any allergies, food sensitivities, or medical conditions that could affect your child's participation.

  • Allergy Policy Agreement
Aspire makes every effort to maintain a safe environment; however, we cannot guarantee that our facilities or materials are free from allergens. Parents must disclose all known allergies or medical conditions during registration. By enrolling your child, you acknowledge that Aspire staff cannot monitor all student interactions with food or materials and agree to assume responsibility for allergy-related risks.

  • SECTION 5 — Behavioral Expectations

    Aspire Learning Community Expectations
Aspire operates as a collaborative learning community built on respect, cooperation, and responsibility. Students are expected to follow teacher guidance, participate respectfully, and contribute positively to the learning environment.

Because our classrooms are intentionally small and highly interactive, students must be able to participate without requiring specialized behavioral accommodations.

Students who repeatedly disrupt the learning environment, refuse to follow instructions, or behave in ways that compromise the safety or experience of other students may be withdrawn from the program at the discretion of Aspire administration.

  • SECTION 6 — Bullying Policy
Aspire maintains a zero-tolerance policy for bullying, intimidation, or harassment of any kind.

Students are expected to treat peers and staff with kindness and respect. Any form of bullying, repeated teasing, physical aggression, or exclusionary behavior will be addressed immediately.

Parents will be notified of incidents, and repeated violations may result in removal from the program without refund in order to protect the learning environment for all students.

    Full Behavioral Disclosure Policy

Families must disclose any prior incidents involving bullying, aggression, suspension, expulsion, or behavior-related school changes during the application and registration process.

If Aspire later determines that relevant behavioral history was intentionally withheld or misrepresented, the student may be administratively withdrawn immediately without refund.

  • SECTION 7 — Photo & Media Permission

I grant Aspire STEAM permission to use photographs or recordings of my child in Aspire publications, promotional materials, digital platforms, and other communications representing Aspire programs and initiatives. I understand that once images or recordings have been published or distributed in print or digital materials, they may continue to appear in previously created content and may not be removable.

I allow my child to appear in classroom photos and videos shared with families.

I allow Aspire STEAM to use photos or video of my child for promotional or marketing purposes.

Extended Media Use: I grant Aspire STEAM permission to use images or recordings of my child in publications, promotional materials, digital platforms, and other communications representing Aspire programs and initiatives.

  • SECTION 8 — Payment & Deposit Policy

    Registration Deposit

A non-refundable registration deposit is required to secure your child’s seat in any Aspire program.

Because enrollment is intentionally limited, deposits ensure that reserved spaces are held for committed families.

Deposits are applied toward program tuition but are non-refundable under any circumstances, including schedule changes, withdrawals, or failure to attend.

Step Up For Students (SUFS) Payment Policy
Aspire STEAM Academy accepts scholarship funding through the Step Up For Students (SUFS) program.

To secure enrollment, families are required to submit the Initial Enrollment Payment, which includes the registration fee and the first month’s tuition.

    How Tuition Payments Work
    • The initial tuition payment made at registration will be credited toward the final month of tuition for the academic year.

    • Months 2–10 of tuition are expected to be paid through the Step Up For Students scholarship program.

    • Families are responsible for approving and submitting tuition payments through their SUFS account according to the SUFS payment schedule.

    Parent Responsibility
Aspire STEAM Academy cannot manage, guarantee, or control scholarship disbursements made through Step Up For Students.

    Parents are solely responsible for:

    • Maintaining an active SUFS account
    • Ensuring scholarship funds are available
    • Approving tuition payments within the SUFS portal
    • Resolving any issues related to scholarship disbursement

Any concerns regarding scholarship eligibility, payment timing, or funding availability must be addressed directly with Step Up For Students.

    Delayed or Missing Scholarship Payments
If scholarship funds are delayed, unavailable, or declined for any reason, the parent/guardian remains financially responsible for tuition payments.

Failure to maintain tuition payments may result in suspension of services or administrative withdrawal, in accordance with Aspire STEAM Academy policies.

    Scholarship Changes
If a student’s scholarship status changes, is revoked, or becomes inactive during the school year, families remain responsible for fulfilling all tuition obligations associated with enrollment.

Aspire STEAM Academy partners with Step Up For Students to make tuition accessible; however, families remain responsible for managing and approving scholarship payments through the SUFS system.

Enrollment at Aspire STEAM Academy does not guarantee scholarship funding.

  • SECTION 9 — Tuition & Withdrawal Policy
Aspire programs operate on a reserved-seat model. Tuition secures your child’s place within a small learning community and covers staffing, materials, and program preparation.

If a family chooses to withdraw after enrollment, previously paid tuition is non-refundable.

Aspire reserves the right to remove a student from the program if behavioral expectations cannot be met or if participation disrupts the learning environment.

  • SECTION 10 — Parent Partnership Agreement
Aspire operates as a partnership between educators and families.

Parents play an important role in supporting classroom expectations, reinforcing respectful behavior, and encouraging student responsibility.

Families who thrive at Aspire are those who value collaboration with teachers and support the shared goal of creating a positive learning environment for all students.

  • Program Selection:

Enrollment must include payment of the Registration fee and First Month's Tuition.
