Accident Waiver: I hereby grant permission for my child/children to participate in dance classes and related physical activities at iBalance Dance Studio. I acknowledge and accept that participation in dance involves physical exertion and carries a risk of injury.I affirm that my child/children is/are in good physical condition and do not suffer from any condition or disability that would limit or prevent safe participation in this program.By signing this waiver, I release and hold harmless iBalance Dance Studio, its owners, instructors, employees, and staff from any and all liability, claims, demands, or causes of action for injuries sustained by my child/children while on studio premises, or while participating in studio-related events or activities, whether held on or off-site, before, during, or after class..

Medical Wavier Consent: I hereby grant permission for any and all necessary medical attention to be administered to my child/children in the event of an accidental injury or illness, whether it occurs in an emergency or non-emergency situation, until such time as I can be contacted. I understand and agree that I am solely responsible for any and all medical expenses incurred as a result of such treatment. I release iBalance Dance Studio, its owners, staff, and representatives from any liability for the cost of medical care provided to my child/children and acknowledge that the studio and its staff are not financially responsible for any medical treatment.

Photo Release and Agreement : I hereby grant full permission to iBalance Dance Studio and its staff to capture, use, and reproduce photographs and video recordings of me and/or my child/children for promotional purposes related exclusively to iBalance Dance Studio.I understand that these images and recordings may be used in brochures, websites, social media, advertisements, press releases, and other promotional materials created and distributed by the studio. I acknowledge that such images may appear with or without names, and I waive any rights to inspect or approve the finished product or the use to which it may be applied.

Physical Contact: I understand and acknowledge that due to the physical nature of dance instruction, there may be physical contact between instructors, staff, and students during classes, rehearsals, and performances.I recognize that such contact may be necessary for proper technique correction, alignment, spotting, and overall safety during instruction. I understand that iBalance Dance Studio maintains appropriate professional standards and that all physical contact is intended solely to support effective instruction and ensure the safety of all students..

Tuition & Fee's : I hereby agree to the payment schedule and financial terms set forth by iBalance Dance Studio LLC. I acknowledge that I am responsible for all charges incurred on my account until the balance is paid in full.I understand that failure to make timely payments, without prior notification to the studio, may result in further collection actions. A non-refundable registration fee is due upon enrollment and includes one leotard per student. The first month’s tuition may also be required at the time of registration.Following registration, tuition is due on the 5th of each month. A 20% discount will be applied for a second and/or third family member enrolled in the program; this discount does not apply to registration fees, recital fees, or costume fees.I understand that costume fees are separate from registration and tuition and must be paid according to the installment schedule provided during orientation. I acknowledge that tuition is non-refundable and will not be adjusted due to student absences or variations in the number of class weeks per month. Make-up classes are encouraged and will be announced by the studio.I also understand that recital fees and costume charges are billed separately from tuition, and statements for these fees will be provided during the course of the dance program.

However, we may share personal information with trusted vendors who perform services on behalf of iBalance Dance Studio LLC, such as email communication providers. These vendors are given access only to the information necessary to perform their services and are required to use that information solely at our direction and in accordance with our privacy standards.Your account is protected by a password within the “Parent’s Portal.” We encourage you to maintain the security of your personal information by not sharing your password and by logging out of your account after each session.You have the right at any time to opt out of receiving promotional communications. Instructions for opting out are included in every promotional email we send.By using our services, you acknowledge that you have read, understood, and agree to the terms of the iBalance Dance Studio LLC Privacy Policy.

iBalance Dance Studio LLC expects all students to use social media responsibly and respectfully. Students may not use social media platforms in any manner that could reflect negatively on the studio, its staff, or fellow students. Students should be mindful that their online behavior may impact both their personal reputation and the public image of iBalance Dance Studio LLC. Any posts, comments, or shared content that are deemed inappropriate, offensive, or damaging to the studio’s reputation may result in disciplinary action, including possible removal from the program.
