This form is for cancelling EliteCommit & EliteBlue Memberships only or the old style 12 Month Memberships. For EliteFlex Cancellations you can self-cancel these by following the instructions in the Account Management Tab in the Elite Fitness App.

When do you wish for your membership to be terminated?
Please note that memberships terminated immediately will receive no refund if a payment has recently been processed and your access to our classes/gym will be immediately revoked upon termination. If you wish to see out the rest of your current payment month then select "Day before next scheduled payment"
