I agree to abide by all of the rules of this juried exhibit, as found on www.polkcountyheritagegallery.org. I understand that if this form is incomplete, including but not limited to failing to upload images, my submission may be disqualified. I understand that my submission is final, and I can not change or substitute details or image files after submission.

Liability
I hereby release and forever discharge the Polk County Heritage Gallery, Polk County, all sponsoring organizations, and their directors, officers, employees, agents, and volunteers from any responsibility, personal liability claims, loss or damage arising out of or in conjunction with my participation in the Iowa Exhibited XXXIV, which is not responsible for any injury sustained by exhibitors or guests while within the space designated as the Polk County Heritage Gallery.

Further, I understand that I exhibit my art at my own risk without exception or limitations. The artwork I submit may be reproduced with acknowledgement for promotional purposes. I understand that the Polk County Heritage Gallery is not responsible for artwork that has not been removed from the Gallery by 15 days after the end of the exhibit.
