Punjab Warriors Soccer Academy's LTD Soccer Program
Long-Term Development Program: This program is specially designed for the long-term development of the players. The goal is to get the players ready for Club Soccer.
- IMPORTANT INFORMATION
  The schedule will be available in late September or the first week of October.
  Location: School gyms on the south side of the city (Millwoods Area from October to February.
  Time: The younger groups will train between 6- 8 pm for 1 hr, and the older group could train between 6- 9 pm for 1hr on weekdays.
  Once the group spots are filled, we will not be able to accept any players.
  Players born in 2019-2012 are required to wear proper indoor soccer shoes, shin pads, and soccer socks.
  Players born in 2022-2020 are permitted to wear running shoes only, and shin pads are not required.
- SCHEDULE: Training schedule will be provided 2-4 weeks in advance.
  FEE STRUCTURE:
  BORN 2022-2020: $400 FOR 26 TRAINING SESSIONS
  BORN 2019-2012: $480 FOR 26 TRAINING SESSIONS

TERMS AND CONDITIONS: Punjab Warriors Soccer Academy (PWSA) and its staff/volunteers do not assume liability for any injuries incurred while at the camp or on the way to the camp. Parents or guardians should contact their own insurance carrier to get additional insurance for the player, if necessary. As a condition of enrollment, the following disclaimer of liability shall be accepted by the player's parents or guardians.

DISCLAIMER: The player attending the PWSA Soccer Camps does so at his or her own risk. PWSA shall not be liable for any damages arising from personal injury sustained by the camper while on or on the way to and from camp. The camper and his or her parents or guardians assume full responsibility for any damages or injuries that may occur to the player during the session and so hereby fully and forever exonerate and discharge PWSA from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known, anticipated or unanticipated, resulting from or arising out of the player's participation in the camp.

PHOTO RELEASE: I hereby grant permission to PWSA the right to use, reproduce, and/or distribute photographs, films, videos, and sound recordings of my child without compensation or approval rights, for use in materials created for the purpose of promoting the activities of the PWSA.

MEDICAL TREATMENT PERMISSION: I, the undersigned parent/guardian, do hereby authorize the athletic trainer or his designee at the camp to secure any and all necessary medical treatment. I understand that they will attempt to contact the parent before treatment is initiated. If the PWSA cannot reach the parent, I authorize the attending physician to render any and all medical care that he/she deems necessary.

REFUND: REGISTRATIONS ARE NON-REFUNDABLE AFTER 10 DAYS FROM WHEN THE REGISTRATION IS SUBMITTED.
