By submitting this registration form, I also agree that the classes, preliminary rounds, the final, and the award ceremony will be recorded and used by the Utah Massage Championship for future marketing on social media and your image or video recordings may be used as well. Participating in the Utah Massage Championship allows the Utah Massage Championship to use your images and voice indefinitely for marketing and advertising purposes.
