I understand that violating the rules could result in a verbal correction from the judges and/or the Director, up to and including the possibility of expulsion from the event.
By submitting this registration form, I am stating that I understand that my registration fee is NON-REFUNDABLE. I furthermore agree to abide by the rules of the championship (which can be found on the UMC website) and any requests made with in reason by the Utah Massage Championship Staff.
By submitting this registration form, I also agree that the classes, preliminary rounds, the final, and the award ceremony will be recorded and used by the Utah Massage Championship for future marketing on social media and my image or video recordings may be used as well. Participating in the Utah Massage Championship allows the Utah Massage Championship to use my images and voice indefinitely for marketing and advertising purposes. (participant waiver for marketing)
