The Tribe will only accept one application under this submission form per individual. If you have children that also need to apply for a Tribal ID you will be required to fill this form out separately for each child.

All applicants will be required to submit a photo for their application. (If using a cell phone to capture a picture, have someone else take your picture by using the front-facing camera. Selfies will not be accepted.) Please upload photos at the bottom of the form.

Tribal ID Cards are free. However if you require another card after losing the first one a small fee may apply.
