In full consideration of the risk of injury while participating in the Hockey Leage, and for the right to participate in the Hockey League, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily participate in this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Hockey League, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any kind of risks related to traveling to and from as well as participating the Hockey League, which may include but are not limited to, physical or phycological injury, pain, suffering, illness disfigurement, temporary or permanent disability, economic or emotional loss, and death.

Please ensure that you have thoroughly read and understood our league policies and procedures as outlined below.

• The risks of injury and illness (including but not limited to COVID-19) from the activities involved in this program are significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist; and,
• I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation; and,
• I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
• I understand that playing hockey carries inherent risks, and all players are advised to wear proper safety equipment; Helmets, Shinpads, Gloves, Elbow pads, and Athletic Supports are recommended for all levels of play.
• I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless DC Hockey Leagues, Maryland Recreational Sports and Rockville Sports Arena, their officers, officials, referees, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and owners and lessors of premises used to conduct the event (“RELEASEES”), with respect to any and all injury, illness, disability, death, or loss or damage to person or property, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.

Player Code of Conduct: Player responsibilities are simple: be respectful of others, be responsible, play fair, always do your best, and always exhibit good sportsmanship.

I acknowledge that I have carefully read this form and fully understand that it is a release of liability. I expressly agree to release and discharge the trainer or instructor from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action for personal injury or property damage.
