PARTICIPATION WAIVER, RELEASE OF LIABILITY, MEDICAL AUTHORIZATION & MEDIA RELEASE

1. Acknowledgment of Risk
I am the parent or legal guardian of the participant named on this registration and voluntarily authorize my child to participate in the Big Baller Brand x Basketball Politics No Politics Elite Development Camp.
I understand that basketball training, games, conditioning, athletic instruction, strength and agility drills, and all related camp activities involve inherent risks that cannot be completely eliminated.
These risks include, but are not limited to:
Sprains, strains, bruises, cuts, and fractures
Muscle, tendon, ligament, or joint injuries
Head injuries or concussions
Heat exhaustion, dehydration, or overexertion
Injuries caused by contact with other participants or equipment
Permanent disability
Serious injury
Death
I acknowledge that injuries are an inherent part of athletics and may occur despite proper supervision, instruction, and safety precautions.
I voluntarily assume all known and unknown risks associated with my child's participation.
2. Representation of Physical Fitness
I certify that my child is physically fit and capable of participating in a high-intensity athletic development camp.
I further certify that my child has no medical condition that would prevent safe participation or, if applicable, I have disclosed all medical conditions, allergies, medications, or restrictions during registration.
I understand it is my responsibility to consult my child's physician if I have any concerns regarding participation.
3. Release of Liability
To the fullest extent permitted by applicable law, I release, waive, and forever discharge the following parties from claims arising out of my child's participation in the camp:
Big Baller Brand
Big Baller Brand Grassroots
Triple B's Cares Foundation
Basketball Politics
LaVar Ball
Game 6 Sports Academy
All camp directors
Coaches
Trainers
Volunteers
Event staff
Sponsors
Vendors
Independent contractors
Facility owners
Any affiliated partners, employees, representatives, or agents associated with the event
This release includes claims arising from injury, illness, property damage, theft, loss of personal belongings, or any damages occurring before, during, or after participation, except where prohibited by applicable law.
I understand that participants are responsible for their own personal belongings and that the Released Parties are not responsible for lost, stolen, or damaged items.
4. Emergency Medical Authorization
If I cannot be reached during an emergency, I authorize the camp directors, staff, coaches, athletic trainers, or designated representatives to:
Contact emergency medical services (EMS)
Arrange transportation to the nearest medical facility
Authorize emergency medical treatment deemed necessary until I can be contacted
I understand that all medical expenses remain my responsibility.
5. Parent/Guardian Consent
By signing this waiver, I certify that:
I am the participant's parent or legal guardian.
I have read and understand this entire waiver.
I voluntarily allow my child to participate.
I understand the physical nature of this event.
I understand injuries may occur despite appropriate supervision.
I accept these risks on behalf of my child.
6. Registration & Cancellation Policy
I understand that registration reserves a limited camp position.
Accordingly:
Registration fees become non-refundable if a participant is a no-call, no-show.
Cancellations made less than fourteen (14) days before the first day of camp are subject to a 20% cancellation fee, which is non-refundable.
Refunds for cancellations made more than fourteen (14) days before camp will be processed less any applicable processing fees, unless otherwise stated in the registration materials.
Requests related to documented medical emergencies may be reviewed on a case-by-case basis at the discretion of the camp organizers.
7. Photography, Video & Media Release
I grant permission to Big Baller Brand, Big Baller Brand Grassroots, Triple B's Cares Foundation, Basketball Politics, Game 6 Sports Academy, and their partners, sponsors, successors, and assigns to photograph, record, livestream, or otherwise capture my child's likeness, voice, image, and participation during the event.
I understand these materials may be used in:
Advertising
Social media
Television
Livestreams
Websites
Printed materials
Promotional campaigns
Future camp marketing
Educational materials
I understand that no compensation will be provided for such use and that these rights are granted in perpetuity unless prohibited by law.
8. Code of Conduct
Participants and parents are expected to conduct themselves in a respectful and sportsmanlike manner.
The camp reserves the right to remove any participant or spectator whose behavior is deemed unsafe, disruptive, abusive, or detrimental to the event without refund.
9. Governing Law
This waiver shall be governed by the laws applicable in the Province of Ontario and Canada, except where otherwise required by applicable law.
If any portion of this agreement is determined to be unenforceable, the remaining provisions shall remain in full force and effect.
Parent/Guardian Acknowledgment
By signing below, I acknowledge that I have carefully read this waiver, fully understand its contents, understand that I am giving up certain legal rights to the fullest extent permitted by law, and voluntarily agree to all terms and conditions on behalf of my child.
