Mayhem's Home for Wayward Animals - Adoption Agreement

By signing this document you:

agree to provide the animal with proper food, water, shelter, exercise, and veterinary care.

1. I will not tether or chain the animal for extended periods.
2. I agree not to sell, give away, or abandon the animal. If I can no longer care for the animal, I will return it.
3. I understand training is critical and agree to participate in ongoing training as needed.
4. I understand the Rescue has disclosed all known health/behavioral information and does not guarantee the animal in any way.
5. If the animal is not spayed/neutered, I agree to complete the procedure within 90 days of the date this is signed.
6. All adoption fee donations are nonrefundable.
7. I agree to periodic follow-ups from the Rescue to ensure the animal's well-being.
8. I agree to pay the non-refundable adoption fee as discussed.

 

Liability Waiver

I release Mayhem's Home for Wayward Animals from any liability related to the animal's behavior, and damages or injuries are my responsibility and proper training will be ensured if necessary.
