You can book After School Club using this form. Payment will need to be made at the end via stripe or Childcare vouchers at the end of the form.

Our club at Crudwell CofE Primary School runs on Monday to Friday, with two to three lengths available:

Monday to Friday 15:10-16:30    £7.25
Monday to Friday 15:10-17:30    £13.25

You will receive an email confirmation once your booking has been completed.

Please note that this form is to be used for registered parents only. You will need to go back and complete the registration form first if you have not already done so.

If paying by childcare vouchers...

You have the option to either pay the full amount or part of the total with childcare vouchers. Select yes to the question below and only put the amount you intend to pay by voucher anything remaining will be charged on the next page.

Payment needs to be made before your child attends otherwise the club may refuse to take your child on the day.
