Media Release Form for Minor Children
BEAM Venture Institute has my permission to use my or my child’s photograph publicly to promote the BEAM Venture Institute. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee, or other compensations shall become payable to me by reason of such use.

Waiver and Release of Liability
I acknowledge that participation in the BEAM Venture Program involves physical activity and may include certain risks of injury. I release and discharge BEAM Venture Program organizers and staff from any liability for injuries that may occur during the program.

Payment of Fees
I understand that the program’s yearly fee of $690 is split into two equal payments for the convenience of parents. The first installment of $345 is due upon registration, and the second installment of $345 will be automatically charged three months after registration, typically in November or December, depending on the registration date. The first payment is non-refundable. If a student qualifies for competitions such as regional, state, and/or national events additional costs will apply. These expenses, including competition registration fees, travel, accommodations, and meals, are not included in the program fee. Additionaly, student participation in these competitions may require parent or guardian chaperoning. Families can cover these costs directly or support their child through fundraising efforts to help offset the expenses.

Withdrawal Policy
Parents or guardians agree and understand that a written notice of withdrawal must be submitted by December 1st if parents or guardians wish to withdraw the child from the second semester of the program. Notice should be turned in by email to admin@beamventure.org. If notice is received on or before December 1st, parents or guardians will be eligible for a refund of the second semester program fees of $345. Please note that the first semester fee is non-refundable.
