I acknowledge that participation in Minnesota Ballet Theatre auditions and related activities involves inherent risks, including physical injury and exposure to communicable diseases, including COVID-19. I understand these risks may result in serious injury, illness, disability, or death and cannot be fully eliminated.

I voluntarily assume all risks associated with participation and certify that I have no medical condition that would prevent safe participation. I understand that professional physical contact may be part of dance auditions.

I authorize Minnesota Ballet Theatre to seek emergency medical treatment on my behalf and accept responsibility for all related medical expenses.

To the fullest extent permitted by law, I release and hold harmless Minnesota Ballet Theatre, its directors, staff, contractors, and agents from any claims or liabilities arising from my participation, including injury, illness, property damage, or death.

Photo & Video Release

I grant permission for Minnesota Ballet Theatre to photograph and/or record me during auditions for promotional, archival, or informational use without compensation. All media remains the property of Minnesota Ballet Theatre.
