I, the custodial parent or guardian of student of Little Studio Yellow, LLC., in consideration of my child’s participation in Summwe Camps, do hereby assume on behalf of my minor child, all risks inherent in dance and summer camp activities, and accept full responsibility for any and all injuries and accidents which may occur as a result of my child’s participation in camp, and release Little Studio Yellow, LLC and its agents and employees, from all such claims.

I give consent for my child’s picture to be taken and used at Little Studio Yellow, LLC discretion.

TLS will give a full refund to cancelations made 21 days before the start of camp.

TLS will give half refunds to cancelations made 7 days before the start of camp.

Refunds will not be given to cancelations made less than 72 hours before the start of camp.

Camper will only be released to Pick Up Adults listed on registration form.
