Only one student and one class per form. Your email address is specific to you as a student and cannot be used to register persons other than yourself.
Necessary steps:
1: You must have the instructor's approval to audit a class. Email the instructor and copy ccsf.extension@ccsf.edu
2: You must have taken the class the maximum allowable times for credit. Please email us at ccsf.extension@ccsf.edu a copy of your transcript showing your previous enrollment(s) in the class.
3: If you are registered for 10 or more credit units, the fee can be waived, please email us at ccsf.extension@ccsf.edu with a copy of your current class schedule.
Once we receive the instructor approval and transcripts via email, we will respond with a code to add to this form. The code is required to process the form.
Questions with a red star * are required, the others are optional.
You will need to provide:
Your CCSF student ID number
the course name and number CRN#
instructor name
number of units
semester
Once you have completed payment you will be emailed a receipt. Please retain the receipt to show your instructor.
