    Please complete the below form and you will receive an automated email confirmation of registration. We will send a separate email from dfwlrc.health@gmail.com with a link to pay for your order via Zelle (no fees) or credit card (nominal processing fees) If you have multiple dogs, please fill out a form for each dog, but we will send one invoice. If you are not familiar with OFA age requirements for testing or your breed's recommended testing, please go to www.ofa.org. Dogs MUST be able to be restrained without sedative and owners will NOT be allowed in the room for radiographs per OSHA regulations. For echoes, dogs MUST be able to lie on the table comfortably-- please email if you are unsure or need clarification. Once payment is received, no refunds will be made unless the event has to be cancelled for some reason. If you cannot make it and need to transfer to another dog/owner, we can work with you. Please email dfwlrc.health@gmail.com AS OF 5/17/2026 the Echocardiograms are FULL; we are looking into adding another vet or day, but at this time sign ups are for WAITLIST only.   • In order to streamline traffic, each owner will be assigned a window that you'll be scheduled in. If you wish to be assigned the same window as a travel partner/ friend, please enter his/her name in the Other section so we can schedule you together.
