Policies and Agreements
Liability Release
I hereby waive, release and hold harmless Berkeley Center for Dance, LLC its owners and instructors from any and all liability or claims resulting from any injury to me or my child due to participation in any classes or programs offered at or by Berkeley Center for Dance, LLC.
Medical Release
I further grant permission for emergency first aid to be given to my child. I grant permission for my child to be taken to a nearby hospital, and its staff has my authorization to provide treatment which a physician deems reasonably necessary for the well-being of my child.
Media Release
Berkeley Center for Dance, LLC reserves the right to take pictures/video at all programs and events. Video and pictures may be used in any future Berkeley Center for Dance, LLC advertisement or publications.
Payment Agreement
I agree to pay for first and last months tuition fees and each month’s tuition for the school year, for every class for which the student is registered, unless I inform Berkeley Center for Dance, LLC of any changes in writing or e-mail before the end of the previous month. Payment is due on the first of each month. If accounts are paid after the 10th of the month, I agree to pay a late fee of $10.00. If accounts are paid after the 20th of the month, I agree to pay a late fee of $20.00. if accounts are paid after the 30th of the month, I agree to pay a late fee of $30.00. Students will not be allowed to take class if their account is delinquent for more than 60 days. I also agree to pay a returned check charge of $25.00 for any checks returned by the bank. I understand that tuition and costume payments are non-refundable and non-transferable. I understand my account balance must be paid in full to receive costume(s) and participate in the spring recital.
I, the undersigned, have read and agree to all of the above.
