Consent Form and Assumption of Risk Statement
I give permission for emergency medical treatment of the students named above if I cannot be first contacted and a reasonable effort has been made to do so.
In consideration of membership of Myrna Rae’s Studio of Dance, and being allowed to participate in any studio events or activities, the parents/legal guardian of the minor participants named above agree:
1. Participants shall be instructed to follow all Myrna Rae’s instructors’ safety guidelines at all times.
2. I fully understand and will instruct the minor participants that:
A. There are risks and dangers associated with participation with dance/gymnastics events and activities including but not limited to those of bodily injury, partial and/or total disability, paralysis and death.
B. The social and economic loss and/or damages which could result from those risks and dangers can be severe.
C. These risks and damages could be caused by the negligence of participation or the negligence of others.
D. There may be other risks not known to us or are not reasonably foreseeable at this time.
3. I accept and assume such risks and responsibilities for the losses and/or damages following such injury, disability, paralysis or death.
4. I agree that this Consent and Assumption of Risk Statement pertains to each and every event or activity sponsored by Myrna Rae’s Studio of Dance.
5. I hereby authorize any pictures or videos taken during dance class or at any dance event to be published without any further notice.
Myrna Rae’s Studio of Dance Policies and Procedures
REGISTRATION - There is a $50 fee per dancer and a $65 fee per family due at registration.
PAYMENTS - ALL lesson payments are due at the first lesson of each month and are NON-REFUNDABLE in the event of absence, withdrawal, act of God, pandemic or dismissal of a student. There will be a $15 late charge for any payments made after the 15th of each month. Legal Action will be taken for any account 30 days past due. All cost, collection expenses and attorney fees will be paid by the client. A $35.00 return check fee will be added to all checks that are returned by the bank.
ATTENDANCE - Students are expected to be regular and prompt to all classes. Please call or email if your student will be absent. There are no refunds for missed lessons.
CLASS PLACEMENT - The placing of students is at the director's professional discretion.
DRESS CODE - Hair must be secured away from face for all classes. All students should be dressed in clothing that easily allows for movement. NO JEANS! All students should have appropriate shoes for the class they are taking.
CLASS APPAREL - All shoes, tights, and costumes need to be ordered through the studio.
CLASS CANCELATIONS - If Mercer County schools are closed early or canceled for inclement weather, there will be no dance class. Canceled
classes will be posted on our Facebook page, and emailed. There will be no reduction of monthly payments for school closings.
CLASS OBSERVATION - We will not be having any watch weeks this year. This may change later in the year and if so, we will update you as soon as we can. Keep in mind students are more focused and work much harder when it is just the student and instructor.
COSTUMES - Costumes will need to be paid for by February 1st – 5th for recreational dancers. Costumes have to be paid in full before they can be ordered. Costumes will run between $100 and $120 per dance. THERE ARE NO REFUNDS FOR COSTUMES!
COMMUNICATION - Parents are expected to be aware of what is going on. We post studio information, performance dates and times and
important events at the studio. We have a Facebook page that we keep updated and send out emails. If a parent is unaware, it is not our fault. Please pay attention to the signs in the waiting room and make sure we have an updated email address for you. The studio email is myrnaraesdance@gmail.com. RECITAL - Our annual recital will be in June. Since costumes are already ordered and dances are finished, it is expected that the students be at the recital performance. It is appreciated that all students stay for the whole show. We want all the students to have an audience to perform for. Parents are also asked to volunteer at the concession stand if we are able to have one. The recital fee of $25 per dancer will be due with June's tuition.
I understand that the registration fee does not cover class tuition. The first monthly tuition payment is due in September.
