ACKNOWLEDGMENT OF RISKS
I understand that participation in Dance Team Central events, including but not limited to dance camps, clinics, rehearsals, performances, and competitions (collectively, the “Event”), involves inherent risks. These activities may include, but are not limited to:
Stretching, jumping, turning, and high-impact movements
Physical contact with other participants
Use of equipment and performance surfaces
Extended physical exertion and fatigue
I acknowledge that these activities carry risks including, but not limited to:
Sprains, strains, fractures, or broken bones
Head injuries, including concussions
Falls, collisions, or accidental contact
Dehydration, exhaustion, or overexertion
Emotional stress or performance-related anxiety
I further acknowledge that participation may expose me (or my child/ward) to communicable diseases, including but not limited to COVID-19, influenza, or other illnesses, and that such exposure may occur despite reasonable precautions.
ASSUMPTION OF RISK
I voluntarily assume full responsibility for any and all risks of injury, illness, damage, or loss, whether caused by the negligence of Dance Team Central or otherwise, to the fullest extent permitted by law.
MEDICAL REPRESENTATION & CONSENT
I represent and warrant that I (or my child/ward):
Am physically and mentally able to safely participate in the Event
Have no condition that would prevent safe participation or pose a risk to others
I understand it is my responsibility to consult a physician prior to participation if needed.
In the event of an emergency, I authorize Dance Team Central and its representatives to secure medical treatment for me or my child/ward. I agree to be fully responsible for any and all medical expenses incurred.
RELEASE OF LIABILITY & COVENANT NOT TO SUE
To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue Dance Team Central, the event venue, and their respective owners, directors, officers, employees, contractors, volunteers, agents, and representatives (collectively, the “Released Parties”) from any and all claims, demands, damages, rights of action, or causes of action arising out of or related to participation in the Event.
This includes, but is not limited to, claims arising from:
Personal injury, illness, or death
Property damage or loss
Negligence of the Released Parties
INDEMNIFICATION & HOLD HARMLESS
I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, liabilities, damages, costs, and expenses (including attorney’s fees) arising out of or related to my participation, or my child’s/ward’s participation, in the Event.
INSURANCE RESPONSIBILITY
I understand that Dance Team Central does not provide medical, accident, or health insurance coverage for participants. I acknowledge that I am solely responsible for obtaining and maintaining any desired insurance coverage.
COMMUNICABLE DISEASE ACKNOWLEDGMENT
I acknowledge the inherent risk of exposure to communicable diseases in any group setting. I voluntarily assume all risks related to such exposure and agree that Dance Team Central is not responsible for any illness or related consequences.
RULES, CONDUCT & REMOVAL
I agree to comply with all rules, policies, and instructions provided by Dance Team Central and event staff.
Dance Team Central reserves the right to remove any participant from the Event for:
Unsafe behavior
Disruptive conduct
Failure to follow instructions
Removal may occur without refund.
FACILITY, TRANSPORTATION & OFF-SITE ACTIVITIES
I understand that Dance Team Central does not control or guarantee the safety of event venues or third-party locations. I assume all risks associated with travel, lodging, and activities outside the Event itself.
PHOTO & VIDEO RELEASE
I grant Dance Team Central the irrevocable right to photograph and/or record me or my child/ward during the Event. I authorize the use of these materials for promotional, educational, and commercial purposes, without compensation.
All media remains the property of Dance Team Central.
SEVERABILITY
If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
GOVERNING LAW
This agreement shall be governed by and interpreted in accordance with the laws of the State of California.
ACKNOWLEDGMENT OF UNDERSTANDING

I fully understand its contents
I understand that I am giving up substantial legal rights, including the right to sue
I sign this agreement freely and voluntarily
