Submission Checklist:

	Send Cervical and/or Lumbar x-rays only
	Send only Flexion and Extension views
	Submit 1 job order for Cervical and another job order for Lumbar
	File formats accepted are: jpg, jpeg
	Your file size may not exceed 24000mb
	For Cervical views, we must be able to see the anterior and posterior foreman magnum and the spinous process of C7.
	For Lumbar views, we must be able to see the entirety of L1 through L5 and at least half the sacrum.

Please do not confuse the Date of Patient Birth, or the Date of Accident or the Date of the Xray.
All the information below is required for analyzing the Xrays.
