Confirmation Registration Form
If you have a student that would like to make the Sacrament of Confirmation during the 2025/26 faith formation year, please fill out the following registration form.

There is a required Confirmation Meeting for all students and parents of students making the Sacrament of Confirmation on Wednesday, September 17th at 7 PM in the Cathedral social hall.

Please call the parish of Baptism and ask them to send us a copy of their certificate.

If your student was Baptized at a parish in Duluth or has one on file at Stella Maris Academy, I will get the certificate for you.

I understand that the resulting photographs and or video tape footage may be edited, if necessary, and then published and or posted in parish newsletters, website, and other material associated with the East Duluth Catholic Faith Formation ministry.*

Confirmation GroupMe Chat
GroupMe is an app for large group chats. The East Duluth Faith Formation staff will use this app to communicate upcoming events pertaining to Confirmation to the students. Safe environment regulations are being followed with three adults present in the chat. Additionally, the group chat is set to private and only those individuals approved by the faith formation staff are allowed to join. The staff will never contact your child individually through this app, and all events advertised through the app will also be advertised through the parish bulletins and announcements.

Authorization & Consent
I consent to the participation of my child/ward in the above named activity. In consideration for my child/ward's participation, I agree to reimburse and indemnify East Duluth Faith Formation(understood to include the Diocese of Duluth, Stella Maris Academy Holy Rosary Campus, Cathedral of Our Lady, St. John the Evangelist, Saint Benedict Parish, Saint Mary Star of the Sea Parish, Saint Michael's Parish, St Joseph's Parish) for all reasonable legal and court fees incurred by East Duluth Faith Formation in defending a lawsuit that I or my child/ward may bring against East Duluth Faith Formation which relates to the above named activities. If East Duluth Faith Formation is found not legally liable By the courts and prevails in the lawsuit. If East Duluth Faith Formation is found liable for the injuries sustained by the child/ward, this paragraph will not apply.
I certify that I have an understanding of this agreement and the risks and hazards associated with the activity described above that my child ward will be participating in. I further understand that I had the opportunity to fully discuss this agreement with a representative of East Duluth Faith Formation to clarify any concerns or questions about the activity or this agreement that I may have had.
