Waiver

You acknowledge on behalf of yourself, your minor child(ren), and others listed on this registration form that you and others listed on this form assume all risk of personal loss, injury, disability, or death which may result from participation in the Run the Ravine 5K. You certify that you have adequate health insurance to cover any injury or damage caused to yourself or your minor child(ren), or else agree to personally bear the cost of injury or damage. You agree to defend, indemnify and hold St. Croix Lutheran Academy and its representatives harmless from any and all losses, liabilities, claims, damages or other financial harm incurred at the Run the Ravine 5K including reasonable attorneys' fees, as a result of any actions of you or your minor child as well as any injury or damage caused to any other person during your participation and activities at the Run the Ravine 5K.

You also authorize and consent to the use of voice, photograph and likeness as well as that of any minor child(ren) covered by this agreement by St. Croix Lutheran Academy without reservation or limitation. You agree to receive no compensation for this use, and property rights to any photographs or video material produced or prepared by St. Croix Lutheran Academy shall vest in and remain with St. Croix Lutheran Academy.
