If your family has more than one student, please complete one form per locker.

Sponsor/rentee is responsible for providing a combination lock and NPD office must be given combination to be kept on file prior to locking. NPD is not responsible for lost or stolen items left in an unlocked locker.

I understand the renter/student is responsible for providing the lock and the current combination must be kept on file with the office.I understand that in order to keep personal items in the locker for the duration of the rental period, I am required to have a lock on my locker at all times. If my locker is found without a closed lock, it will be assumed that it is day-use only and and all items will be removed and placed into lost and found.I understand that food/drink are not to be kept in my locker overnight.I acknowledge that writing or putting tape, stickers, etc. of any sort on the locker walls, the inside, or outside of the door is not allowed.I understand that If my locker is damaged/vandalized in any way, I will be liable for replacement/repair fees for full unit. This cost is to be determined at time of replacement/repair.I understand that Northern Plains Dance is not responsible for any lost or stolen items.I understand if my locker is not cleaned out by May 31, 2026, the lock will be removed and all belongings will be put in the lost & found.I understand that NPD reserves the right to ask the me to clean out my locker temporarily at any given time for repairs, cleaning, etc. It is my responsibility to clean out my locker promptly.
