Talent LLC, Talent Finders Of Az Hereby Accepts The Above Applicant To Join National Performers Academy In Development Workshops.
As A Performer At The Industry Prep Zoom Workshops, The Following Terms And Conditions Apply:
2. Photo Rights- In Consideration For Current And Future Publicity, I Hereby Grant The National Performers Academy, Including Diana Horner The Right To Reproduce Or Otherwise Use In Perpetuity, For Purposes Including, But Not Limited To, The Unremunerated Use Of Performer’s Image For Advertising And Publicity.
3. Refund Policy: You Must Cancel In Writing By Certified Mail, Return Receipt Requested. If Your Certified Letter Of Cancellation Is Postmarked Within Three Days Of Signing This Agreement, Refunds Paid In Excess Of $695 Will Be Refunded. Once The Scheduled
Workshop Begins And/Or After Three Days Whichever Is First. No Refunds Will Be Issued. (The $695 Registration Fee That Is Non-refundable)
I Have Read The Cancellation And Refund Policy Specified Above, Received A Copy And By Signing Below,
I Agree To Be Bound By This Agreement.
The Issuer Of The Card Identified On This Form Is Authorized To Pay The Amount Shown On This Agreement
As Total Due. I, This Issuer Understands That There Will Be An Agreed Amount Charged To My Account
Until The Balance Is Paid In Full, And Agrees To Pay Such Total Together With Any Other Charges Due Thereon, I, Further Understand . If
Funds Are Not Available At The Agreed Time Of Payment, A Late Fee Of $25.00 Will, Be Added To The Account. The Issuer Understands And Agrees To Pay All Such Total(S) Subject, To And In Accordance
With The Agreement Governing The Use Of Such Card.
As The Issuer Of This Card I Have Read, Understood, And Agreed To The Cancelation And Refund Period
In Accordance With The Terms And Conditions Governing National Performers Academy Agreement.
I, The Issuer Of This Card Agree That Any Disputes Arising From My Agreement With National Performers
Academy For Cancelations, Refunds, Or Chargebacks To My Card Agrees, That I Will; In ‘good Faith’ Try
To Resolve These Disputes With National Performers Academy Before Contacting My Credit Card Issuer Or Bank.
I Hereby Release National Performers Academy And All Subsidiaries, Affiliates And Licensees Therefore, From All Claims, Demands, Liabilities And Obligations Whatsoever Arising Out Of The Use Of Such Portraits Or Pictures By National Performers Academy And Subsidiaries, Affiliates And Licensees Therefore.
I Understand That There Are No Promises Or Guarantees Of Work Or Representation.
National Performers Academy Is A Development Company Offering Coaching, Advice And Guidance For The Entertainment Industry.
