SPL Bookplate Dedication Form

Need a gift? You can purchase a personalized bookplate to dedicate a book in “honor of” or in “memory of” your loved one for a donation of $25 to the Friends of the Sandwich Public Library.

Choose from one of 5 book categories (Adult Fiction, Adult Non-Fiction, Young Adult, Children's Fiction, Children's Non-Fiction) and we'll do the rest! The Honoree (or the designated family member) will be notified by the Friends of the Sandwich Public Library of your thoughtful gift, as soon as the order process has been completed.

Your gift will help enhance the Library's physical collection and will be enjoyed by fellow Library Patrons for years to come.

We thank you for your donation and continued support of the Sandwich Public Library.

The Friends of the Library is a registered 501(c)(3) non-profit organization.
