Thank you for your interest in the Minnesota Ballet School Pre-Professional Day Program.This intensive training program is designed for dedicated dancers ages 14+ who are ready to pursue advanced classical ballet training through the Vaganova Method and prepare for future opportunities in dance.Admission is by audition only. Audition Options: In-Person Audition (scheduled by appointment)Video Audition (for students unable to attend in person) Please complete the audition form below. Participation Risks

Participation in this audition involves physical activity and the inherent risks associated with dance training, including, but not limited to, injury or illness. By participating, you acknowledge these risks and agree to follow all instructions, safety guidelines, and training directions provided by Minnesota Ballet School (MBS).

Acknowledgement of Risk & Waiver of Liability

I confirm that I do not have any physical condition that would prevent my safe participation in dance activities.

I agree to participate in activities at Minnesota Ballet School (MBS) and/or Minnesota Ballet Theatre (MBT), use required equipment appropriately, and travel to and from additional dance-related activities as needed.

In the event of an emergency, I understand that I may be transported to the nearest medical facility or the preferred hospital listed by me, if deemed necessary by emergency responders. I acknowledge that I am responsible for any medical treatment, transportation, or related expenses.

I understand that professional physical corrections and hands-on adjustments may be a necessary part of ballet and dance instruction.

Photo & Video Release

I grant permission for photographs and videos taken during Minnesota Ballet School activities to be used for educational displays, promotional materials, social media, advertising, and other school-related purposes.

I understand that photographs and videos taken by Minnesota Ballet School remain the property of Minnesota Ballet School.

Dance training and participation involve physical demands. Injury may occur if activities are performed improperly or without following proper instruction and safety guidelines.
