* This form is a request to be registered. Once availability and eligibility are confirmed, you will be notified of registration and any payment will be accepted.
* If you need to reschedule your course for another start date, you must notify CMPAC at least two (2) business days in advance of the start date for the current course.
* Attendance at all classes is mandatory for certification. Any unexcused absence will be considered an automatic incomplete for the course.
* Please arrive at least 15 minutes early. Anyone arriving after the class has begun will not be admitted and will be considered as not attending.
* All fees are nonrefundable and nontransferable. Failure to complete a course will result in the forfeiture of any and all fees.
* Any incomplete or failed course must be re-taken in its entirety. Any fees associated with any failed or incomplete course must be re-paid in order to re-register for the course.
* Repeated absences from registered courses will result in the suspension of course-taking privileges.
* Labs are required for many courses in order to complete the certification process. Failure to complete the required number of Labs in the specified time period will result in a course failure.
