- I understand that participation in classes, workshops, events, and activities organized by the Minnesota Ballet Theatre & School involves physical activity that may result in injury, including travel to and from training sessions. I authorize emergency medical care for myself or my child(ren) if needed.
- I am aware of the contagious nature of COVID-19 and other illnesses, and voluntarily assume the risk of potential exposure while participating in school activities. I understand that such exposure could result in personal injury, illness, permanent disability, or death.
- I release, indemnify, and hold harmless the Minnesota Ballet Theatre & School, its staff, agents, and employees from any claims of liability for personal injury, death, or property damage arising from participation in classes or activities.
- I understand that tuition and fees are non-refundable unless otherwise stated in the school’s refund policy.
- I give permission for photographs, videos, and audio recordings of myself or my child(ren) to be taken during classes or activities. I understand these recordings may be used by the school for promotional purposes, social media, website, and advertising, and that all media is the property of the school. I understand that no compensation will be provided.
