**Liability Waiver & Release**
By signing below, I acknowledge that participation in the HERStory in the Making Basketball League, operated by the Bradshaw Family Foundation, involves physical activity and inherent risks, including but not limited to injury, illness, or accidents.

 
I voluntarily allow my child (or myself, if over 18) to participate in all league activities, including practices, games, and related events. I assume full responsibility for any risks, injuries, or damages that may occur as a result of participation.

I hereby release and hold harmless the Bradshaw Family Foundation, HERStory in the Making, its staff, volunteers, coaches, officials, partners, and facility providers (including BCYF centers) from any and all claims, demands, or causes of action arising out of participation in this program, except in cases of gross negligence.

I certify that the participant is physically able to participate and has no medical condition that would prevent safe participation. In the event of an emergency, I authorize program staff to seek medical treatment if necessary.

Code of Conduct Acknowledgment

I agree to uphold sportsmanship, respect officials, staff, and fellow participants. I understand that failure to do so can result in removal from all league activities now and in the future. 

I understand that participation is contingent upon acceptance of this waiver and agreement 

I, the parent/guardian of the participant listed above, give permission for my child to participate in the Araion Bradshaw Basketball Camp field trip to the Skating Rink (11 Rhoades St., Dorchester Center, MA 02124) on Thursday, July 9th from 1:00 PM–3:00 PM. I understand that campers will walk approximately 15 minutes to and from the location under staff supervision and that skate rental/admission costs are expected to be approximately $10–$15 per participant and agree to pay for this cost. I acknowledge that participation in skating and off-site activities involves inherent risks, including but not limited to slips, falls, injuries, transportation/walking-related incidents, or other unforeseen circumstances. By signing below, I voluntarily assume these risks and release, waive, and hold harmless the Bradshaw Family Foundation, Araion Bradshaw Basketball Camp, its staff, volunteers, partners, and affiliates from any liability, claims, injuries, damages, or expenses arising from participation in this field trip, except in cases of gross negligence or willful misconduct. I certify that my child is physically able to participate and agree that camp staff may seek emergency medical care if necessary.
