GREENVILLE GUARDIAN YOUTH SPORTS ASSOCIATION
FALL BASEBALL PARTICIPATION WAIVER & RELEASE
I, the undersigned parent or legal guardian, acknowledge, understand, and agree to the following:
1. Assumption of Risk
I understand that participation in youth baseball and related activities, including practices, games, training sessions, camps, travel, and use of facilities, involves inherent risks. These risks include, but are not limited to, physical injury, sprains, fractures, head injuries, illness (including communicable diseases such as MRSA, influenza, and COVID-19), permanent disability, paralysis, and death.
I understand that these risks may arise from my child’s actions, the actions of other participants, coaches, or spectators, the condition of facilities, equipment used, or other causes.
2. Voluntary Participation
I knowingly and voluntarily allow my child/ward to participate in Greenville Guardian Youth Sports Association Fall Baseball activities and freely assume all risks, both known and unknown, even if arising from the negligence of the Organization or others, to the fullest extent permitted by law.
3. Release and Waiver of Liability
On behalf of myself, my spouse, my child/ward, and our respective heirs, assigns, personal representatives, and next of kin, I hereby release, waive, discharge, and hold harmless Greenville Guardian Youth Sports Association, its directors, officers, commissioners, coaches, officials, volunteers, employees, sponsors, agents, other participants, and any owners or lessors of facilities used for activities (“Releasees”) from any and all claims, demands, causes of action, or liabilities arising out of or related to my child’s participation in Fall Baseball activities, whether arising from negligence or otherwise, to the fullest extent permitted by law.
4. Indemnification
I agree to indemnify and hold harmless the Releasees from any and all claims, damages, losses, or expenses (including attorney fees) arising out of or related to my child’s participation, even if caused in whole or in part by the negligence of the Releasees, to the fullest extent permitted by law.
5. Rules, Conduct, and Safety
I agree that my child/ward will comply with all Greenville Guardians rules, safety requirements, and instructions provided by coaches, commissioners, and officials.
I understand that:
Unsafe, unsportsmanlike, or disruptive behavior may result in removal from participation
Only approved players and coaches are allowed on the court or bench areas
Parents and spectators must follow all conduct expectations set by the Organization
If I observe any unsafe condition or concern regarding my child’s readiness or the activity itself, I will immediately remove my child from participation and notify an appropriate official.
6. Medical Acknowledgment
I affirm that my child/ward is physically able to participate in Baseball activities and has no medical condition that would prevent safe participation, or that such condition has been disclosed to the Organization.
7. Acknowledgment to Child
I affirm that I have explained to my child/ward:
The risks involved in participation
Their responsibility to follow all rules and instructions
The importance of safe conduct and sportsmanship
8. Media Release
I, the undersigned parent or legal guardian, hereby grant permission to Greenville Guardian Youth Sports Association (“GGYSA”) to photograph, video record, and otherwise capture the image, likeness, and voice of my child/ward in connection with participation in GGYSA activities, including but not limited to practices, games, tournaments, camps, and events.
I understand and agree that these images and recordings may be used by GGYSA for lawful purposes, including but not limited to:
Social media posts
Website content
Promotional materials
Marketing and advertising
Printed materials such as flyers, banners, and programs
I acknowledge that:
No compensation will be provided for the use of such images or recordings
GGYSA retains full rights to use, edit, reproduce, and distribute such media
Media may be used in both current and future promotional efforts
I understand that GGYSA will not identify my child by full name without additional consent; however, first names, team names, or general identifiers may be used.
I waive any right to inspect or approve the finished product in which my child’s likeness appears and release GGYSA and its representatives from any claims, liabilities, or damages arising from the use of such media.
This release is granted in perpetuity and applies to all media formats now known or later developed.
9. Agreement and Understanding
I have read this agreement, fully understand its terms, and understand that I am giving up substantial rights by signing it. I sign it freely and voluntarily without inducement.
