Photo/Video Release

I hereby grant the Pretty Fit Fitness LLC permission to use my likeness in a photograph, video, or other digital media ("photo") in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of the Pretty Fit Fitness LLC and will not be returned.

I hereby irrevocably authorize the Pretty Fit Fitness LLC to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge the Pretty Fit Fitness LLC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY THEIR SIGNATURES BELOW. I ACCEPT:

Liability Waiver for Physical Exercise

I understand that physical exercise and training involve inherent risks, including the risk of injury, illness, or accident. I voluntarily assume all risks associated with participation in any fitness activity, class, workout, or related exercise program offered by Pretty Fit Fitness LLC.

I release and hold harmless Pretty Fit Fitness LLC, its owners, instructors, employees, and representatives from any and all claims, liabilities, or demands arising from my participation, except where prohibited by law.

BY SIGNING BELOW, I ACKNOWLEDGE THAT I HAVE READ, UNDERSTAND, AND AGREE TO THIS LIABILITY WAIVER FOR PHYSICAL EXERCISE.

MONTH-TO-MONTH MEMBERSHIP & CANCELLATION POLICY

By enrolling, you acknowledge and agree that this is a month-to-month membership with automatic recurring billing. Your first payment will be charged on the date you sign up, and subsequent payments will be automatically charged on the same calendar day each month.

You may cancel your membership at any time. To cancel, please email info@getmovingwithelena.com or text 813-843-4018. To avoid being charged for the next billing cycle, cancellation requests must be received before your next scheduled payment date. Cancellation stops future billing, and payments already processed are nonrefundable.
