Purpose of Precamp
Precamp is a childcare environment provided for leaders’ children during camp activities. It is designed to support leaders while maintaining a safe, structured, and supervised environment for young children.
Medical Authorization (Child & Leader)
I certify that I (and my child/children, if applicable) am in good health and able to participate in camp activities.
In the event of an emergency, I authorize Nebraska NxtGen Ministries, its Director, and designated staff to secure and authorize emergency medical treatment for myself and/or my child, including transportation, hospitalization, treatment, surgery, and/or medications, as deemed necessary by licensed medical professionals.
Every reasonable effort will be made to contact me or my emergency contact prior to treatment. However, if I cannot be reached, I authorize such care as deemed necessary.
I understand and agree that I am financially responsible for any medical expenses incurred.
Parental Responsibility & Supervision Agreement
I understand that while Precamp provides supervised childcare during designated times, I remain ultimately responsible for my child’s care and behavior.
I agree to:
-Check my child in and out during designated Precamp hours
-Be available and reachable at all times while my child is in Precamp
-Immediately respond if contacted by Precamp staff regarding my child
-Not leave my child unattended outside of designated Precamp care times
-Inform staff of any medical needs, allergies, behavioral concerns, or special instructions
-I understand that failure to follow these expectations may result in my child being removed from the Precamp program.
Assumption of Risk & Liability Release
I understand that participation in camp and Precamp activities involves inherent risks, including but not limited to play activities, recreational environments, and interaction with other children.
I voluntarily assume all risks associated with my child’s participation and my own participation.
I agree to release, waive, and hold harmless Nebraska NxtGen Ministries, the Nebraska Ministry Network, The Crossing Retreat Center, and their respective directors, officers, employees, agents, and volunteers from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, loss, or damage, except in cases of gross negligence.
Precamp Conduct Expectations
I understand that my child is expected to:
-Follow instructions from Precamp staff
-Treat other children and leaders with respect
-Participate appropriately in activities
I understand that behavior that is disruptive, unsafe, or harmful to others may result in removal from Precamp.
Media Release
I grant permission for Nebraska NxtGen Ministries and the Nebraska Ministry Network to use photographs, video, and audio recordings of me and/or my child for promotional, educational, and ministry purposes in any medium, without compensation.
